At Dentistry of West Bend, we offer several Dental Membership Plans that help you save money on dental care. These in-house dental discount plans let you decide when to visit your dentist and how often. There are no restrictions on when to schedule your routine dental exams and no waiting periods. Your membership entitles you to save on all preventive, basic, and major procedures for a low annual fee. Memberships cannot be financed through CareCredit.

Terms & Conditions

Membership is effective on the date in which payment is received and terminated the last day of that month the following year.
Payments are due when services are rendered.
Accepted payment forms for service fees are cash, check and credit card.
Membership benefits include dental services at 15% (cash or check) and 12% (credit card) discount for those services not included in the membership package purchased. This is not an insurance plan and can only be used at Dentistry of West Bend, LTD.
The Dentistry of West Bend, LTD Membership Plan is not transferable and cannot be used with any other dental coverage. It is the sole responsibility of the member to maximize their benefits by arranging all of the appropriate appointments within the 12-month membership.
Whitening products, Sonicare toothbrushes, replacement toothbrush heads, Chlorhexadine, MI Paste and other dental products are not included and there is no cost reduction for these products.

Plan Options

Child Plan (0-13)

Our Child Plan is Suitable for patients age 13 and younger, and includes the following set of services yearly:

  • 2 Regular Dental Exams
  • 1 Emergency Dental Exam
  • 2 Professional Cleanings
  • 2 Children’s Fluoride Treatments
  • 2 Oral Cancer Screenings
  • 2 Pediatric Periodontal Screenings
  • 1 Set of Bitewings
  • 1 FMX or Panoramic X-ray (1 Every 3 Years)
  • 15% discount on any additional treatments (cash or check)
  • 12% discount on any additional treatments (credit card)

You may also purchase a plan for multiple children.

Adult Plan (14+)

For our adult patients with healthy teeth and no periodontal disease, we offer a plan that provides the following yearly services:

  • 2 Regular Dental Exams
  • 1 Emergency Dental Exam
  • 2 Professional Cleanings
  • 2 Oral Cancer Screenings
  • 2 Periodontal Screenings
  • 1 Set of Bitewings
  • 1 FMX or Panoramic X-ray (1 Every 3 Years)
  • 15% discount on any additional treatments (cash or check)
  • 12% discount on any additional treatments (credit card)

 

Perio Plan (14+)

The Perio Plan is specifically designed for our patients with symptoms of periodontal disease

  • 2 Regular Dental Exams
  • 1 Emergency Dental Exam
  • 3 Perio Maintenance Visits
  • 2 Oral Cancer Screenings
  • 2 Periodontal Screenings
  • 1 Set of Bitewings
  • 1 FMX or Panoramic X-ray (1 Every 3 Years)
  • 15% discount on any additional treatments (cash or check)
  • 12% discount on any additional treatments (credit card)

 

Limitations and Exclusions

  • There is no coverage outside the office of Dentistry of West Bend, LTD, as this plan is only valid at Dentistry of West Bend, LTD. NO discount will be given for specialty referrals, even if the referral was initiated by Dentistry of West Bend, LTD. 
  • Dentistry of West Bend, LTD Membership Plan is not liable for hospitalization of hospital charges of any kind for any services or dental care which are covered under workman’s compensation, medical insurance, general liability, etc. 
  • If the appointments are not used, the member is not entitled to a refund. Membership rates may be reviewed and adjusted on an annual basis with notification. 
  • Dental fees are subject to change at any time 
  • All appointments must be cancelled or rescheduled with a 48-hour notice to avoid cancellations fees and/or lose discounts or benefits of non-used hygiene appointments.
